Verändern eine verbesserte Mundhygiene und die Anwendung eines niedrig dosierten topischen Fluorids das Erscheinungsbild der Läsionen nicht, können Anwender auf andere Maßnahmen, wie die Zahnaufhellung und die Mikroabrasion, zurückgreifen. Dieses Mal stammen unsere *Clinical Tips* vom Clinical Advisor für Endodontie und Zahnaufhellung bei Ultradent Products: Dr. Renato Miotto Palo. Die mangelhafte Bildung von Zahnschmelz kann verschiedene Ursachen haben. Störungen dieser Art können die normale Lichtreflexion auf dem Zahnschmelz verändern. Dadurch wird ein weißer Fleck (White Spot) erzeugt. ![Clinical_Opalustre_Dr.Jessop_8_17_18](https://de.ultradent.blog/hs-fs/hubfs/Clinical_Opalustre_Dr.Jessop_8_17_18.jpg?width=400&height=136&name=Clinical_Opalustre_Dr.Jessop_8_17_18.jpg) „Jede Beschädigung einer glasartigen oder prismatischen Oberfläche führt zu einer weißen Stelle (ähnlich wie Kratzer auf Uhrenglas). Wenn eine gestörte Schmelzbildung vorliegt, wird weißes Licht reflektiert. Das sind White-Spot-Läsionen“, erklärt Dr. Palo. Visualisieren Sie das Gleichgewicht von Farbe und Transluzenz „Liegt eine Veränderung vor, beginne ich bevorzugt mit der minimal-invasivsten Behandlungsmethode und verstärke die Maßnahmen nach Bedarf“, so Dr. Palo. Dieser Ansatz gibt den Patienten Sicherheit und sorgt für die maximale Erhaltung der Zahnsubstanz. „Es ist besser“, so Dr. Palo weiter, „mit einem Zahnaufhellungsgel zu beginnen und dann die Ergebnisse zu prüfen. Anschließend kann ich mich bei Bedarf für eine Mikroabrasion entscheiden. Hellt das Whitening Gel die Zahnsubstanz auf, sind die weißen Flecken weniger wahrnehmbar. Sie passen sich dem Erscheinungsbild der Zähne an.“ Zu einem anderen wichtigen Thema rät Dr. Palo: „Schon eine geringfügige Dehydrierung aufgrund einer Zahnaufhellung kann einen bestehenden White Spot noch auffälliger machen. Die Patienten befürchten, dass dies auf das Zahnaufhellungsgel zurückzuführen ist. Allerdings kann dieser Effekt durch eine Rehydrierung wieder rückgängig gemacht werden.“ Daher sollten Patienten zunächst über eine Zahnaufhellung versuchen, die White-Spot-Läsionen zu maskieren, bevor sie sich für eine andere Behandlungsmöglichkeit entscheiden. Ein harmonisches, ästhetisches Lächeln hängt von zahlreichen Faktoren ab. Aus diesem Grund ist es für die Patienten besser, ihre Zähne von einem Zahnarzt aufhellen zu lassen, der über ein Gespür für Ästhetik verfügt. „Zähne bestehen hauptsächlich aus zwei Dingen: Dentin und Zahnschmelz. Die Zahnfarbe ergibt sich aus der Kombination dieser beiden Bestandteile. Normalerweise ist Dentin dunkler und Zahnschmelz heller. Jüngerer Zahnschmelz ist milchig, älterer ist transluzent. Älteres Dentin ist dunkler und jüngeres heller. Die Zahnfarbe ist eine Kombination aus Lichtbrechung und -reflexion, welche auf die Zahnsubstanz trifft“, erklärt Dr. Palo. Wann sollte man sich für eine Mikroabrasion entscheiden? Hat die Zahnaufhellung bei einigen White-Spot-Läsionen nicht das gewünschte Ergebnis erzielt, kann eine Mikroabrasion durchgeführt werden. Dabei wird eine dünne Schicht der defekten Schmelzprismen entfernt, um eine normale Lichtreflexion und ein normales Aussehen wiederherzustellen. Sie ist für die Entfernung oberflächlicher Schmelzfehler geeignet, einschließlich leichter Verfärbungen durch Fluorose. Sie hat u. a. folgende Eigenschaften: •    Ergebnisse in einer Sitzung •    6,6 % Salzsäure für minimal-invasive Veränderungen durch eine chemische Entfernung der Flecken •    Siliziumcarbid-Mikropartikel sorgen für eine sanfte mechanische Abrasion Zusammenfassend besteht die Strategie darin, mit einem Zahnaufhellungsgel zu beginnen. Durch die aufhellende Wirkung wird ein Farbton erzeugt, bei dem weiße Flecken kaum erkennbar sind. Während der Zahnaufhellung selbst wirken die White Spots möglicherweise markanter. Wird die Behandlung aber fortgesetzt, sind sie weniger wahrnehmbar. Erfordert der Fall weiterführende Maßnahmen, wird eine Mikroabrasion durchgeführt. Dabei wird eine dünne oberflächliche Schicht des veränderten Zahnschmelzes entfernt, damit die betroffenen Zähne das Licht wieder normal reflektieren und eine normale Farbe annehmen.
